Appointments

Professional services are provided by appointment. To schedule an appointment, call the Center at (310) 988-1970 or fill out our online Contact Us form

Translators

If the person to be examined does not speak or read English, please tell the receptionist when you schedule the appointment so that we can have a translator available, as well as appropriate reading materials in order to evaluate the usefulness of the low vision devices we recommend.

Transportation

Our wheelchair-accessible van and our station wagon provide door-to-door transportation to and from the Center for optometric appointments, diabetes groups, independent living skills classes, and support groups and counseling sessions, with prior arrangement. We also provide referrals to other transportation programs. If you need transportation, it is necessary to tell the receptionist when scheduling your appointment.

Funding and Service Fees

The Center is a 501 (c) 3 agency (Fed Tax ID #95-3771974), governed by a 20-member Board of Directors. Funding for the Center comes from foundations and corporations, individual donations, service fees, the California Department of Rehabilitation, the City of Santa Monica, and Beach Cities Health District. For information about making a donation, please see How to Help Us.

Service fees are on a sliding scale based on ability to pay. The Center is a participating provider of Medicare, Medi-Cal, and the California Department of Rehabilitation. These programs may cover a portion of the services provided by the Center. There is no charge for many of our rehabilitation services. Project VIP provides low-cost or no-cost vision rehabilitation services for people who are low-income or under-insured. Call Audrey Valenzuela at (310) 988-1970, for eligibility requirements.
